Please note that this form is required for enrollment in services with HOH but filling out / submitting the form is not a commitment to services on your part nor a commitment of immediate availabilty on our part.

Current clients and mentorship boarders must see that any visitors have a signed liability on file when they visit the farm to interact with any horses-- even personal horses-- boarded at the farm.

Click the picture above to see a template of our adoption agreement. We are licensed by the GA Dept of Ag and a part of our license is having such an agreement to go with our horses. Every adoption is tailored to the specific horse, but this gives an idea of what the adoption agreement will look like. Bear in mind, the purpose of the agreement is not to in any way invade your privacy or rights as an owner, but simply to be an assurance that the adoption will begin with all pertinent information in place and the horse will always be safe and cared for. HOH wants to always be a resource and support for any horse owner, but especially owners of HOH horses!
